Children Born Abroad
If you are a U.S. citizen and wish to register your child born in Poland as a U.S. citizen, you must apply for a Consular Report of Birth Abroad (CRBA). You may apply for your child’s CRBA at the Embassy in Warsaw, at the Consulate General in Krakow, or at the Consular Agency in Poznan. All three locations are by appointment only.
For information about standard procedures and required documents, as well as the forms you need to fill out, please visit the Department of State’s central CRBA web page. Please note: you must bring your child’s long-form birth certificate (Odpis Zupełny Aktu Urodzenia).
If you have additional questions about CRBAs, please contact American Citizen Services in Warsaw (ACSWarsaw@state.gov or call +48 22 504 2784), in Krakow (KrakowACS@state.gov or call +48 12 424 5100), or in Poznan (firstname.lastname@example.org or call +48 61 851 8516).
