I have a visitor’s visa that is still valid for 4 years. If the passport which has the visa has expired, do I need a new visa in my new passport or can I travel to the U.S. with both my old and new passports?

No, you do not need a new visa. You may travel to the U.S. with BOTH your expired passport containing the valid visa and your new passport. However, please make sure that when your old passport was canceled by the Polish authorities your visa was not accidentally canceled as well. If there are holes punched through the page containing the visa then the visa is no longer valid for travel and you will need to apply for a new visa.
