How to apply for a transportation letter?Applicants must apply in person for transportation letters. Applications for transportation letters are accepted at the Immigrant Visa Section of the U.S. Embassy in Warsaw between 1:30 pm and 3:00 pm, Monday through Friday, except for United States and Polish public holidays.
You should bring: ▪ your valid passport; ▪ three identical visa photographs; ▪ evidence that you are a Lawful Permanent Resident and have been outside the United States for less than one year. ▪ a police report showing the circumstances of the loss or theft of the green card (if applicable).
In order to facilitate processing your case, please bring a completed lpr Status Confirmation Questionnaire with you when you come to apply for your transportation letter.
Is there fee for a transportation letter?The fee for issuing a transportation letter is $165. The fee is payable at the consular section only, in U.S. dollars or the equivalent in Polish currency at the Embassy's exchange rate; in cash or by a credit card (Visa, MasterCard, American Express, Novus/Discover or Diners only). Checks are not accepted.
Note: Applicants from non-E.U. countries may only pay with cash.
When will I be able to return to the U.S. with my transportation letter?It may take several days to process the transportation letter. Because transportation letters are valid only for a maximum of 30 days, your intended trip back to the United States must be within 30 days of the day of issuance.
I am a permanent resident and my baby was just born in Poland.Your child can enter the United States with a transportation letter provided that: ▪ the child was born during the mother’s temporary visit abroad ▪ his/her admission to the U.S will be within two years of birth; and ▪ either accompanying parent is applying for readmission upon first return after the birth of the child.
You will need to follow all of the procedures outlined above to apply for the transportation letter.
You should bring: ▪ your baby’s valid passport; ▪ three identical visa photographs of your baby; ▪ evidence that you are a Lawful Permanent Resident and have been outside the United States for less than one year. ▪ A complete version of your child's birth certificates (Odpis zupełny aktu urodzenia) with an English translation by a sworn translator (tłumacz przysięgły). The Polish "short form" of birth certificate is not accepted.
