- A-1 or A-2 (Foreign Government Officials)
- C-3 (Foreign Government Officials in Transit)
- G-1, G-2, G-3 or G-4 (International Organizations)
- NATO 1-6 (NATO Officials, Representatives and Employees)
- Travelers age 80 or over with no prior refusals within the last two years.
- Children under the age of 14 whose parent has a valid American visa or is a citizen of a country included in Visa Waiver Program (VWP). See "Applications for Children" for more instruction.
These applicants may submit their documents to the Service Window Monday through Friday 8:30am to 12:00noon, except for legal Polish and American holidays and the last business Friday of the month, or may send their documents by the DHL Courier Service (see Sending visa applications by a courier service).
Visa applicants that do not require an interview must submit the electronic application form DS-156 with a bar-code and present all the documents described in Required Documents.
Applicants for A, C-3, G and NATO visas do not pay application fee and do not require form DS-157. They are requested to provide a diplomatic note or letter on official stationary from the appropriate delegating institution. The Embassy requires a request from the Chief of the Transportation Section at the U.N. Secretariat for officials and employees of the U.N. and their immediate families.
The Service Window is located on 12 Piekna Street at the Embassy in Warsaw. Processing of applications filed at the Service Windows takes several days. Passports with visas will be delivered by DHL courier company within 5 working days, except for official visas which can be picked up in the Information Window from 2:00 to 3:00 pm upon earlier arrangement.
