Medical Treatment

Medical treatment in the United States


An applicant whose primary reason for traveling to the United States is for medical treatment or evaluation can come for an appointment on Mondays through Fridays 8:00am – 12:00pm, except Polish and American holidays. These applicants should submit the electronic application form DS-156 with a bar-code and present all the documents described in Basic documents and photographs.

In addition the applicant must provide:


  1. A medical report/statement from a physician stating the nature of the medical condition and the possibility of treatment in Poland;
  2. A statement from a licensed American physician, on official letterhead or hospital letterhead, describing:

    a. the medical problem based on documents sent from Poland;
    b. the treatment required;
    c. the cost of the treatment, including the worst-case scenario;
    d. the estimated time the treatment will require, including the worst-case scenario;
    e. the name of the person the physician believes will pay the cost of therapy or treatment;
    f. the relationship between the physician and the institution where the treatment will take place; and
    g. how much of the treatment is in-patient and how much is outpatient.

    This letter from an American physician on official letterhead must be sent directly to the Non-Immigrant Visa Section in Warsaw at fax number 011-48-22-504-2532. Before coming for an interview please confirm receipt of the fax by phone (011-48-22-504-2000).
  3. If relatives in the United States are paying for travel and medical expenses, evidence of sufficient funds to cover these expenses should be provided.
  4. If the applicant is paying, the applicant should provide evidence of financial resources in Poland.

All visa applicants, even those going to the United States for medical treatment, must prove that they are not intending immigrants.
