Medical Treatment

Medical treatment in the United States

If you are planning to travel to the United States primarily for medical treatment, you will still be required to set up an appointment and bring in all required documents.  In addition to all of the required documents, you will be asked to provide: 

  1. A medical report/statement from a Polish physician stating the nature of the medical condition and the possibility of treatment in Poland;

  2. A statement from a licensed American physician, on official letterhead or hospital letterhead, describing:

    • the medical problem based on documents sent from Poland;

    • the treatment required;

    • the estimated time the treatment will require, including the worst-case scenario;

    • the name of the person who will pay the cost of therapy or treatment;

    • proof of how the person identified as paying will cover the estimated costs (e.g. six month bank account history, statement from insurance company, etc);

    • confirmation from the care providers' billing offices that their requirements regarding the applicant's ability to pay for the envisioned treatment have been satisfied.  This could conceivably involve multiple care providers (the treating physician, the hospital facility, diagnostic providers, etc.)

The letter from an American physician on official letterhead must be sent directly to the Non-Immigrant Visa Section in Warsaw at fax number 011-48-22-504-2532. Before coming for an interview please confirm receipt of the fax by phone (011-48-22-504-2000). 

Applicants going to the United States for medical treatment must demonstrate that they are qualified for a visa.
